Adobe Acrobat Document PPTC Registration Pack Contents and Timetable 2011-2012
  This gives details of all the documents in the registration pack and the timetable of the key dates.
Adobe Acrobat Document PPTC REGISTRATION FORM 2011-12
  This should be completed and returned to the school which you have nominated as Assessment Centre 1 by 4.00pm on Friday 23rd September 2011.

  These will assist in the completion of the Registration Form and other documents.

  This is a sample of the Mathematics questions being used for the Entrance Assessment.

  This is a sample answer sheet being used for the Entrance Assessment.
Adobe Acrobat Document PPTC Access Arrangements Form AA1
  This should be completed if you are claiming Access Arrangements for your child (see Guidance Notes).
Adobe Acrobat Document PPTC Access Arrangements (Medical Information) Form AA2
  This should be completed by your child’s General Practitioner or Medical Consultant in support of a claim for Access Arrangements in relation to a medical condition.

  This should be completed by a suitably qualified person in support of a claim for Access Arrangements.

  You should consult this policy if you are considering applying for Access Arrangements or Special Circumstances for your child.

  You should consult this policy if, following the Entrance Assessment you are considering applying Special Circumstances for your child.
Adobe Acrobat Document PPTC (Form SCR) Registering a Claim for Special Circumstances
  If you wish to Register a Claim for Special Circumstances Form SCR should be completed and returned to the Assessment Centre by 16th December 2011
Adobe Acrobat Document PPTC (Form SC1) Applying for Special Circumstances
  If, having received the Entrance Assessment outcomes you wish to apply for Special Circumstances Form SC1 should be completed and attached to the Transfer Form along with supporting evidence. Please read the relevant Admissions Criteria section(s) of the ELB “Transfer Guide for Parents”
